We support family members and caregivers under the Colorado Health First DD,CES, and SLS Waiver programs

Hire

We hire family members and friends as Independent Contractors to be PAID caregivers for their loved ones

CES/SLS Homemaker Basic and Enhanced
$19/$28 hr

Performance of household living tasks

SLS Personal Care
$24 hr

A range of basic life assistance to the individual

CES/SLS Respite $23 hr

Short-term care due to absence of the individual's normal care provider

Supported Community Connector (SLS) - Based on support level

Assisting the individual with accessing typical activities and functions in the community

Community Connector (CES)
$37 hr.
 

Enable the individual to access typical activities and functions of community

SLS Mentorship
$40 hr

A mentor promotes self-advocacy to the individual
